Severe traumatic brain injury  was got ten by a boy in a car accident 04.04.10. He was treated at the city hospital in Ust-Ilimsk with a diagnosis of severe concomitant injury. Brain contusion with severe compression of epidural hematoma, closed fracture of the right femur with displacement, damage to abdominal organs.The disease was complicated by postoperative abscess in the removal of a kidney, brain swelling, soft tissue necrosis of the right tibia, bilaterally hypostatic aspiration bronchopneumonia. The child underwent several operations: removal of necrotic tissue, installation and removal of metal constructions, excision of deep bedsores of the right shin with the removal of the lateral gastrocnemius muscle of the abdomen, followed by the imposition of tracheostomy withdetubation, the imposition of gastrostomy.

In6 months after the injury, the child was enrolled at  the Institute of Emergency Children's Surgery and Traumatology, where he has been performed the operation to close the (plastic) bone defect  of the left parietal-temporal region. Next hospitalization at the Research Institute of Emergency Children's Surgery and Traumatology followed in 8 months after the injury to carry out a ventriculoperitoneal shunt. After that, the child, within five years, was  hospitalized several times in different hospitals to perform a plastic surgery of Achilles, neurolysis of the peroneal nerve, subcutaneous Achilles tenotomy of Bayer  and rehabilitation.

In five years after the trauma , the child was noted to have a psycho-organic syndrome.He responds on inspection, carefully examines, differentiates close peple, periodically interested in toys, has the short interest in toys. He understands said speach, but understanding is reduced, expressive speech in the form of individual sounds.

 

Playing consists of two hands, tetraparesis is more marked in the right extremities, contracture of the right ankle, wasting of leg muscles on the right. Self-flips, siting down, crawling on all fours, with reliance on the hands, can stand in support, walks with support of the body, gait paretic relying on equine-varus-adduct right foot. There are deployed generalized seizures.


In a joint charity campaign of the Foundation of  assisting  in providing medical and social rehabilitation and care for children with severe trauma and its consequences (The Fund  of Children's Dr. Roshal) and Johnson'®s Babycompany  thefunds were raised for the rehabilitation of children in the Institute of Emergency Children's Surgery and Traumatology. As a result of this action, Matvei (from2.02. to 26.02.15) was undergoing rehabilitation at the Research Institute of Emergency Children's Surgery and Traumatology. The child was made a full diagnostic system, evaluated rehabilitation opportunities, a program of rehabilitation. Under this program, for 4 weeks, the interdisciplinary specialists  of the rehabilitation team were intensive working, correcting cognitive and motor impairment. As a result of the rehabilitation, the child’s gait, balance, reaction, attention are improved, the orthopedic shoes are matched.
